Application Process

• First: Complete the On-Line Pre-Application

• Then: Complete and mail, in a single packet (documentation MUST arrive in one package by March 1, 2005 ):

1. The Agreement Form , completed and signed
2. Your Statement of Merit with optional accompanying supporting documents selected by you. In one typewritten page, describe why you are the best candidate for this scholarship, based on academic merit and your activities relating to the field of life sciences. Supporting documentation may include such items as letters of recommendation from employers or community organizations.
3. A letter of recommendation from a college or university professor
4. The following documentation:
   • Proof of your parent's or spouse's loss in action:
     • Death certificate showing cause of death
     • Military: Report of Casualty Form DD 1300
       Or
       Civilian death certificate
     • MIA: appropriate U.S. documentation
   • Proof of relationship to the person killed in the line of duty
     • For children :
       Certified copy of you birth certificate showing the parent's name
       Or
       certified copy of the decree or order of adoption showing that name)
     • For spouses:
       Marriage certificate
       And
       proof that you were married at the time your spouse was lost
     • Transcripts from the last 2 years of your college or university studies, showing coursework and declared major.
